Missed calls during consultations: options compared

Published 6 min read

Small clinics miss calls in predictable ways: the only person who could answer is in a consultation, at the counter or already on the phone. The right fix depends on how many people can answer, what your phone system can do and what callers need when they ring.

Quick answer

Start with the phone system you already have. Ring groups and a queue with a callback option help when staff are busy for short periods. When nobody is free for longer stretches, forward unanswered calls after a set number of seconds to an answering service or AI receptionist that can book or take a structured message. Voicemail alone leaves callers without an answer until someone checks it.

Why do calls go unanswered during clinic hours?

Missed calls during opening hours usually trace back to a small number of causes. A sole practitioner cannot answer while with a patient. A single receptionist cannot answer while checking someone in or while on another line. Some systems ring only one handset, so a busy desk means an engaged tone. Without a queue, callers who ring during a busy minute get no answer at all and may not try again. Your phone reports, or a two-week tally, show which of these is happening and at what times, which tells you more than the total number of missed calls.

  • Practitioner in session with no one else to answer
  • One receptionist covering phone and counter
  • Calls ringing a single handset
  • No queue, so busy periods produce engaged tones
  • Breaks and meetings leaving the desk empty

What can your existing phone system already do?

Check what your current system can do before buying anything. The 3CX manual, as one example, describes ring groups that ring members at once or in order, queues that hold callers until someone is free, a maximum queue wait after which calls go to another destination, and queue callbacks that let a caller hang up and be called back. Callbacks can be triggered on request, automatically after a timeout, or offered after a set time. Office-hours rules can route calls differently during breaks and meetings. Ask your provider which of these features your plan includes and whether they are switched on.

How does call forwarding work for a mobile-based practice?

Some sole practitioners run the practice number on a mobile. Mobile networks offer several forwarding types. Telstra, for example, lists unconditional forwarding, which sends every call elsewhere immediately; forwarding on no answer, which applies if you do not answer within 15 seconds; forwarding when unreachable, if the phone is off or out of coverage; and forwarding when busy, if you are already on a call. A practitioner can switch on unconditional forwarding during sessions, or leave no-answer and busy forwarding on all day, sending those calls to an answering service, AI receptionist or colleague. Check your own carrier's options and charges.

When does a callback queue work, and when does it fail?

A callback queue works when staff have quiet periods soon after the busy one, so callbacks happen within minutes. It struggles when the busy period lasts all morning: callbacks pile up behind new calls, and patients called back an hour later may have booked elsewhere or be unavailable. 3CX, for example, treats a callback the caller does not answer as a failed callback. Track the number of callbacks requested, the time taken to call back and the number that fail. If failures grow, the callback queue is hiding a capacity gap that needs more cover or an overflow service.

What should an overflow service be able to do?

Forwarding calls to someone who can only promise that the clinic will call back moves the queue without shortening it. Before choosing an answering service or AI receptionist, check what it can complete on the first call and how messages reach your team. If it uses AI, tell callers so; the OAIC's guidance on AI products says organisations should be transparent about their use of AI. Give it the same practice information and emergency direction your staff use, and make sure every message lands in a shared queue with an owner.

  • Book, change or cancel appointments in your practice software, or take a structured request
  • Answer routine questions from approved practice information
  • Give emergency direction to triple zero (000) and follow your urgent protocol
  • Send messages to a shared practice queue instead of one person's mobile
  • Answer in the practice's name and disclose AI use where it applies

How do you choose between the options?

Use your call data to describe the gap: how many calls go unanswered, in which intervals, and what callers wanted. If unanswered calls cluster in short bursts and staff free up quickly, ring groups and a queue with callback may be enough. If a sole practitioner is in sessions most of the day, or bursts last longer than staff can absorb, forwarding to an answering service or AI receptionist covers the gap. Ask each provider to quote against your actual call numbers. Yes AI's published plans are on the pricing page, and the free 30-minute clinic assessment looks at your calls and practice software.

Practical checklist

  • You know how many calls go unanswered in each hour, from phone reports or a tally.
  • The number of seconds before a call forwards is agreed and written down.
  • Queue messages state the options accurately, including how to leave the queue.
  • Callback requests have an owner and a target time, and failed callbacks are reported.
  • Messages from any overflow service arrive in a shared practice queue.
  • Every recorded message and overflow script directs emergencies to triple zero (000).

Common questions

Is voicemail acceptable for a clinic?

It can work for a practice with very few calls, provided the greeting gives emergency direction to triple zero (000), says when messages are checked and offers another way to book. The drawback is that every message waits for someone to listen and call back, and some callers will not leave one. Compare the number of voicemails left with the number of missed calls.

Can we use overflow only during business hours?

Yes. Forwarding on no answer and on busy applies whenever you set it, and office-hours rules can limit it to opening hours or to session times. After-hours calls can go to a separate message or service. Write down which arrangement applies at which times so staff know where a caller was sent if the patient asks later.

How many seconds should a call ring before it forwards?

It is a practice decision that balances the chance of a staff member answering against how long callers will wait. Some services fix it: Telstra's mobile forwarding on no answer applies after 15 seconds. Business systems often let you set it; in 3CX ring groups the ring time is configurable. Pick a value, watch unanswered and forwarded call counts for a few weeks, then adjust.

Will callers know their call has been forwarded?

A forwarded call should be answered in the practice's name with the same opening information callers would hear from your staff. If an AI system answers, it should say so early in the call. If calls are recorded, the recording notice should be given wherever the call is answered. Check a sample of forwarded calls each month to see whether they end with a booking or a clear next step.

What does an overflow service cost?

Pricing structures differ between providers, so compare them on the same basis. Ask each provider for a quote based on your actual unanswered call numbers and the tasks you want completed, such as booking into your practice software. Yes AI's published plans are on the pricing page, and anything outside them is quoted after a short scoping conversation.
